We Love Meeting New Patients

We want to thank you for taking the time to visit our website. We are so glad you found us! Here at Align Spine and Pain, we strive to make you feel welcome, safe, and at-home from the moment you step into our office. We’re committed to being your partner and advocate to identify and target the root cause of your discomfort. We look forward to meeting you!

New Patient Checklist

When you come for your initial visit, please bring the following:

  • Driver’s License or a valid government-issued ID
  • Insurance card
  • Reports of previous imaging (X-rays, MRIs, CT scans)
  • List of medications
  • Previous office notes from your primary care provider and/or referring physician
  • Primary care provider contact information

Please arrive 30 minutes prior to your appointment time to allow time for check-in and new patient forms. We know your time is valuable and we will make every effort to minimize your wait time. Arriving early helps us so much and we sincerely appreciate it. In order to minimize wait times, appointments will need to be rescheduled if over 10 minutes past the scheduled appointment time

Occasionally, situations beyond our control come up and we fall behind. Waiting is never fun. If we’re running behind, please know that we are working to get back on track. Thank you for your patience and understanding.
