New Patient Checklist
When you come for your initial visit, please bring the following:
- Driver’s License or a valid government-issued ID
- Insurance card
- Reports of previous imaging (X-rays, MRIs, CT scans)
- List of medications
- Previous office notes from your primary care provider and/or referring physician
- Primary care provider contact information
Please arrive 30 minutes prior to your appointment time to allow time for check-in and new patient forms. We know your time is valuable and we will make every effort to minimize your wait time. Arriving early helps us so much and we sincerely appreciate it. In order to minimize wait times, appointments will need to be rescheduled if over 10 minutes past the scheduled appointment time
Occasionally, situations beyond our control come up and we fall behind. Waiting is never fun. If we’re running behind, please know that we are working to get back on track. Thank you for your patience and understanding.
Insurances Accepted
We accept all major commercial insurance carriers, Medicare, and many others. Major commercial insurances include BCBS, United, Cigna, Aetna, Humana, and Tricare. We are also in network with Medicare, Optum VA, and other government insurance plans. Please visit our Insurances page to learn more.
Payments
All co-payments should be collected prior to the appointment. We accept all major credit cards (Visa, Mastercard, American Express, Discover), debit cards, check, HSA and FSA. There is a $30 insufficient funds fee for rejected checks. Please visit our Payments page for online payments and payment plan options.
Appointment Changes
Things come up and we understand that! To cancel or reschedule your appointment, please call our office during business hours at least 48 hours prior to your appointment time. Appointments are considered late 10 minutes after the scheduled appointment time. If late, you may either reschedule or wait to be seen the same day if your provider has schedule availability.
